A report's found Counties Manukau DHB's mental health services didn't provide a woman proper care

She was admitted to a Community Mental Health Service acute respite facility after a long history of self-harming.
She was later found dead in her room.
Deputy Health and Disability Commissioner Vanessa Caldwell's ruled the facility, and the DHB, breached Consumer Rights under the Code.
Caldwell says shortcoming were found in plans for the woman's crisis management and care - as well as a lack of clinical care.
In response to recommendations, processes have been changed.
