Failings have been found in the care of a woman's foot wound in the weeks before she died

Aged Care Commissioner Carolyn Cooper has found a care home and a registered nurse breached the Code of Health and Disability Services Consumer Rights.

Weekly photos of the woman's wounds were not taken, records were confusing and the referral system to a more specialised care was unclear.

The care home's made several changes since the incident.
