Waikato DHB says it's made a raft of changes since the death in March 2015 of mental health patient Nicky Stevens

Coroner Wallace Bain has formally found that the death of the 21-year-old was as a result of suicide.
His body was found in the Waikato River three days, after he went missing during an unsupervised break from Waikato Hospital's mental health unit.
DHB interim chief executive Derek Wright extends his sincere sympathy to Nicky Stevens' family.
He says there are now tighter restrictions on patient leave, and a stronger emphasis on family involvement in patient recovery.
