The five-year-old was allegedly killed in 2015 by his stepfather, James Roberts - who died before a trial.
At an inquest, Dr Claire Doocey says in 2012 Leon had a lacerated tongue, found to be accidental.
Two years later, injuries from a trampoline accident were listed as 'unclear'.
The hospital saw him next in 2015, with the fatal injuries.
Doocey says staff relied on his parents, because Leon had autism and couldn't speak for himself.
She says he didn't understand consequences so could have potentially put 'himself' in positions of harm
