A medical centre and surgeon have been found responsible after a woman found a swab had been left inside her four days after an operation

The Health and Disability Commissioner Anthony Hill found theatre nurses didn't follow proper protocol in counting swabs used during the surgery.
Hill says the surgical count-in and count-out process ensure a patient's safety - and is required policy.
He says this is an example of how crucial good communication is in an operation theatre.
