A Transport Accident Investigation Commission's report found the capsizing of a water taxi in Stewart Island could have been avoided in 2019.
Chief Investigator of Accidents, Harald Hendel, says this operator has since taken on better weather forecasting to avoid steep and unpredictable seas.
He says the skipper's in a better position to turn back if conditions worsen.
Hendel, says the water taxi's safe pontoon design failed to prevent it from being swamped.
