Coronial findings into the death of five-year-old Leon Jayet-Cole highlighted a complete lack of communication between the DHB, social workers and police -- after a string of violent incidents.
The autistic child died after being rushed to hospital with a serious head injury inflicted by his violent stepfather in 2015.
Canterbury DHB's Acting Chief Medical Officer, Dr Richard French, says staff could have done better -- as Leon and another child in the house frequently needed hospital care.
French says the DHB's committed to sharing clinical information with other government agencies.
