A woman has needed her uterus removed after a routine procedure went wrong in 2017

The Deputy Health and Disability Commissioner has found a gynaecologist gave the woman inadequate services and was in breach of Code for a failure of care.
The woman visited the doctor to get treatment for heavy menstrual bleeding and contraceptive options.
During the procedure, her uterus was injured - but the complications weren't documented.
The woman then had contraction-like pain and ultimately needed her uterus, fallopian tube and part of her colon removed.
