The Health and Disability Commissioner says the man had a public hospital CT scan to investigate abdominal pain.
The radiologist didn't notice indications of a perforated bowel.
The man was discharged and died days later from septic shock.
The report states early treatment might have been prevented.
The radiologist has undergone training and audits, and will write an apology to the man's family.
