A coroner is endorsing a WorkSafe safety alert about managing strangulation hazards on playground equipment, following the death of four-year-old Aldrich Viju

The toddler died at Angels Childcare Centre on Auckland's North Shore in November 2016 after being found with a cord from a toy stilt around his neck.
Coroner Sarn Herdson recommends the alert from 2017 should be considered for revising and republishing - and the Ministry of Education notified of any updates.
The Coroner's office will also ensure SafeKids Aotearoa is aware of the safety alert, regardless of whether it is revised or republished later.
