Mandeep Singh died working at Glaziers Choice - trading as Stake Glass - in January, 2019.
WorkSafe Investigation Manager Steve Kelly says the death could've been avoided if the man was supervised, and had the pack lifter been stored without a load.
The company must also pay 110 thousand dollars to the victim's family.
