The pilot and safety pilot - both senior flying instructors - were killed in the crash.
A Transport Accident Investigation Commission report found it was a controlled flight into terrain accident.
Commission chief investigator Harald Hendel says this happens when an airworthy plane is inadvertently flown into terrain.
He says in this case it's likely the pilot had no idea how close they were to the hills - and the onboard terrain proximity awareness system screen was either dimmed or not selected.
