A Waikato doctor who overlooked a woman's terminal cancer says he was tired, and may not have read the report

Health and Disability Commissioner Morag McDowell says the woman was let down by a lack of communication, and delays in in reading X-ray results.
The ED clinician missed an abnormal mass that was described in the patient's X-ray report in Waikato Hospital in 2017.
She wasn't diagnosed with lung cancer for three more years, and has since died.
McDowell's recommending an audit of all reports acknowledged by the doctor, who's also been asked to apologise to the woman's family.
