A senior Doctor breached the Health and Disability Services Code, over the death of a woman in 2018

She was incorrectly diagnosed, and prescribed indigestion medication -- and the Coroner determined the cause of death as an aortic dissection.
The woman was admitted to hospital with chest pain and shortness of breath, and discharged with medication -- but readmitted the following day, and died in Intensive Care.
Deputy Commissioner Carolyn Cooper says she's not satisfied the doctor took appropriate steps to rule out aortic dissection.
She says the doctor didn't use available supervision, or seek cardiology advice.
