Emerald Waiari Tai and her three-day-old son Tanatui Samuels were found dead in March 2020, hours after leaving hospital against medical advice.
Coroner Erin Woolley says both had sepsis but it's unclear if the baby also died from suffocation while sleeping under his mother.
The Health and Disability Commission accepts there were no red flags in the mother's monitoring, but suggests a dedicated maternal sepsis guideline might've prompted more consideration.
