    A Coroner's found the failure of a baby's cot was the sole cause of his death


    Open Justice reports the accidental death was caused by circumstances the parents weren't likely to be aware of.

    The seven-month-old was placed in the cot about 8pm on the night of the tragedy, wrapped in his usual muslin cloth.

    Its sliding rail came out of its track - and trapped the baby next to the mattress, where the mother found him the next morning.

    Coroner Seung Youn says the baby's mother followed general safe-sleeping practices.

    He says more messaging around cots' structural integrity should be included in Ministry of Health guidelines.
