Open Justice reports the patient had nerve damage that took months to heal, the dentist agreeing she had had a 'terrible experience'.
The Health and Disability Commissioner has found there was no discussion of the force required in the procedure, or explanation of potential risks.
The woman was given no aftercare advice, and had ongoing pain.
The dentist has since completed an ACC claim for nerve injury, refunded the woman's fees and apologised in writing.
