A breach of the Health and Disability Services and Consumers' Rights code - after a swab was left inside a woman after surgery

The woman developed a host of symptoms post-discharge from Health New Zealand's Manukau Super Clinic.

Deputy Health and Disability Commissioner Vanessa Caldwell found a clear failure, with a swab not accounted for during the surgery.

She says she was encouraged by Health New Zealand's prompt response, apology and review.

Caldwell's recommendations include a random audit, and training.
