    The Director of Mental Health is launching his own probe, into how an 11-year-old was mistaken for a missing adult inpatient


    The girl was restrained and injected with anti-psychotics at Waikato's Henry Bennett Centre - after being mis-identified last month

    Health New Zealand has admitted major failings in a review this week.

    Mental Health Minister Matt Doocey, says Director of Mental Health Dr John Crawshaw, felt it necessary to conduct his own inquiry - something he feels is right.

    He hopes Crawshaw, along with the Health and Disability Commissioner, consider the matter seriously and take appropriate action.
