Deputy Health and Disability Commissioner Rose Wall says the mother wasn't told her baby's growth had slowed.
She says when the planned home birth couldn't go ahead, the midwife referred to the hospital's on call obstetrician as "Mr Slice and Dice" - which caused worry.
When the woman went to hospital, the midwife told her the baby was okay - when it was in distress.
Commissioner Wall has recommended the Midwifery Council consider whether the midwife is competent to continue her role.
