Five-year-old Leon Jayet Cole died in 2015 -- from severe head injuries inflicted by his step father, James Roberts.
The coroner says police, CFYs, and the CDHB all failed to see that Roberts -- who had a violent history and was with Leon every time he was hurt -- was to blame for a raft of injuries before the death.
NZ Herald reporter, Kurt Bayer, says Leon first came to the attention of authorities in 2013 -- when his tongue was lacerated.
He says other children in Leon's household were seen with bruising on their face, and taken to hospital for head injuries.
