Nadine Tomlinson and Angus died when trapped in a tractor that slid into a pond on their Otago farm - in Palmerston - in 2018.
The experienced driver was collecting a dead animal.
Her son wasn't wearing a seatbelt.
Coroner Sue Johnson says she believed Tomlinson only applied one brake pedal after becoming concerned about their speed.
She agrees with Worksafe that farmers need to be more careful about the risks to themselves and their children.
