A report's found Hutt Valley DHB failed to flag a man's allergy before he died

The man died of anaphylaxis after being given an antibiotic for an injury on holiday in 2019 - despite his allergy becoming evident in a visit three weeks earlier to the DHB.
The Health and Disability Commissioner found the DHB failed to properly record the man's allergy to the antibiotic, or flag it with his GP.
Deputy Commissioner Dr Vanessa Caldwell says better communication. could have prevented his death.
Health authorities have been urged to apologise to his family, and improve staff processes.
