A man died at Nelson Hospital in 2018 after a surgeon performed a colonoscopy, not realising he only had one lung

The Health and Disability Commissioner has found the surgeon and Nelson Marlborough DHB failed in the man's care.
The patient in his 60s, had to be intubated after suffering a peri-respiratory arrest during the procedure.
He also suffered a perforation in his colon - and later died.
Commissioner Morag McDowell says if the surgeon and health board had communicated effectively, staff would have known the man's lung had previously been removed to treat lung cancer.
