A pharmacist has been found in breach of Health and Disability Services Consumers' Rights - for not checking the correct medication was dispensed

A woman was prescribed an acne medication not recommended for people planning to become pregnant - and warned of the dangers.
But the pharmacist gave out a different medication, which requires patients to avoid pregnancy for three years.
The woman took the wrong medication for 22 days - and the error was picked up when she returned for a repeat.
The pharmacist has accepted full responsibility.
