The man, under the influence of meth, was arrested last June and taken to the Auckland Custody Unit -- where an ambulance was arranged to assess him.
When the ambulance was delayed, officers took the man to hospital but he became unresponsive in the car and was unable to be resuscitated.
Acting Superintendent Sunny Patel says this was a tragic incident.
He says they've implemented further recommendations made by the Independent Police Conduct Authority around wider policy and training considerations.
